Primer fail or something else?

I was shooting my Encore Saturday, black horn 209, CCI Mag primers and had something happened I've never seen before. 1st shot in the bulls eye, 2nd shot 1" to the right and pretty sure that was me. The 3rd shot sounded different and I noticed a lot of blow back around the breech plug area and fouled the area up pretty bad. This area stays pretty clean and I've never had an issues with the fire hole getting plugged. The shot was no where on target that I could see. I wiped the area out, reloaded, bulls eye the next 2 shots. Wondering if maybe I had a bad primer? Same box of primers I've been using the last 2 years. Any ideas?

Could be. I know there was some smoke that came out around the breach/hammer area which I've never seen it do before. It wasn't like a delay or anything just a different noise and more smoke around the chamber area with that black sooty looking residue left behind.
