I'd say an arrow wound that hit the shoulder blade and didn't penetrate .
Arrow would be my guess if it's a shot. Also have quite a few agreeing with Backyardtndeer that it more than likely is just from a deer doing deer things and banging its shoulder on something it shouldn't have

Wart fell off.
My 1st thought would be a wart that he scratched it off going under a fence or something.
Since he had a limp then I would say he might have got gouged with an antler. I seen a small buck 2 years ago that I could tell was gouged with an antler on his back hip. Had a piece of skin hanging off showing red meat. He acted just fine tho.
