I couldn't submit TAFKAP Jr.'s application until I had a SSN in hand. I don't recall a copy of birth certificate being a requirement.

On another note, you may want to discuss it with your brother. I know you want it to be a surprise, but it may already be in the works for him. My brother was disappointed that I did it, but I thought it to be a more appropriate thing for me to give my son, than for my brother to do it.

Just got my daughters last week. Printed the form from twra website. Faxed the form, copy of her birth certificate, and a copy of my drivers license. Very easy and received it very fast.
