I am not sure of all of the requirements. I don't know if a SS# is required for the youngest age bracket or not. Please give our license sales div. a call at (615) 781-6585 and they can walk you through the process.

Walmart used to have the paper applications also, just a heads up.

At the bottom of the page is a link to the pdf paperwork to fill out.
Says for underage person, must provide birth certificate and parents/guardian must provide proof of residency.
