Most likely happened during the caping process. In order to get as small as incision as possible on the back of the head you either chisel out the skull plate or saw the muzzle off.
I usually make a cut between antler burrs then about a 3" cut from the back of one antler between the ears toward the back of the skull. Then cape everything off the skull, if I can't slid the cape over the back of the skull then I will saw the muzzle off and slide forward. This makes for a nicer mount and saves time when sewing up the incision after mounting.
