I am looking for a form to show that i qualify to use a crossbow. It has to be signed by a physician. This is for an out of state bow hunt. Thanks for any info.
Unless something has changed since I left, TWRA no longer has such a form since there are no longer any health related qualifications needed to use a crossbow in TN. You can confirm that with the TWRA Boating and Law Enf. Div. at 615-781-6580 (M-F).
I would suggest contacting the state you are traveling to and using one of their forms.
