Either he's a cactus rack (never sheds velvet), or he damaged both pedicles from his antlers coming off too early (usually from sparring or rubbing in winter).

Had one like that a few years ago. As a 3 1/2 year-old, he had clusters of spikes for both antlers. As a 4 1/2, he just had giant 12-14" spikes on both sides.
