Had a Tri-Star 12 ga actually blow up.
My son-in-law test fired it on my gun range. When he pulled the trigger, he felt something hit his arm. The bolt blew back and completely blew out the side by the magazine. His forearm was bloody and bruised. Tri-star sent us a new 12 gauge.
