Here is my oil change regimen. Oil change. put sticker on windshield with +3000 miles. Drive Notice that I am getting close to mileage on sticker. Think....hmmmm need to change oil soon. Drive Look at sticker and notice that I am now approx 1000 miles OVER what is on the sticker. Think.. I really need to change my oil. Drive Look at sticker and see that I am almost 2000 miles OVER what is on the sticker... Think "Aiiieieee! I need to change my oil NOW!" Drive about 200 more miles. Finally change oil at about 5000 miles. Repeat process. 
Hey, that's my oil change process, too! Give or take a 1000 miles or so.

Flushing the tranny isn't a bad idea if you've never done it.

I have 188k on my truck. Is it worth the extra money to get the high mileage oil change? Or is it just a gimmick? I've read that it doesn't matter, just as long as you keep the oil changed every 3000-5000 miles.
I've put it in my truck since I turned 75,001 miles. I'm not sure of the difference it may have made. It still burns clean at 135,000

I change oil in the X5 BMW every 15,000 miles, have not had a problem

Why try to fix something that isn't broke..........stick to what you have been doing. The most important thing is fresh oil/filter, often.

I've taken two trucks to 300,000 miles. Just change the oil when it's dirty.
